Work Environment

The Family Nurse Practitioner will work onsite at our assisted living facility in Brooklyn, NY, providing care during standard business hours from Monday to Friday, 9 AM to 5 PM. Our facility is equipped with modern medical equipment and resources, ensuring that our team has the tools necessary to deliver outstanding patient care. As a member of a dedicated healthcare team, you will collaborate closely with physicians, nursing staff, and specialists to provide comprehensive care to our residents.

Key Responsibilities

• Conduct thorough assessments of patients to identify healthcare needs and establish individualized care plans.
• Diagnose and manage common and complex medical conditions in patients, particularly in pediatric populations.
• Administer vaccinations, conduct routine health screenings, and provide preventative care recommendations.
• Monitor patient growth and development, documenting changes and discussing concerns with families to ensure optimal health outcomes.
• Refer patients to specialists and coordinate follow-up care as necessary to maintain continuity of care.
• Develop educational materials and resources for families, promoting health awareness and preventive strategies.
• Engage in collaborative treatment planning with a multidisciplinary team to achieve the best outcomes for our patients.

Patient Population / Client Focus

This position serves a diverse patient population, including infants, children, adolescents, and young adults. Your role will be to assess and address a wide range of healthcare needs, from routine check-ups and immunizations to the management of chronic health conditions. Qualifications

• Master of Science in Nursing (MSN) with Nurse Practitioner designation from an accredited university.
• Active New York State Advanced Practice Registered Nurse (APRN) License.
• Board certification as a Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP).
• Current registration with the Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) for prescription writing.

Preferred Experience

• A minimum of one year of experience in primary care or a pediatric clinical setting, where you actively diagnosed and treated patients.
• Proven ability to work effectively within a collaborative team of healthcare professionals to enhance the quality of care.
• Fluency in Spanish is highly desirable but not required; candidates are encouraged to demonstrate communication skills that enhance patient engagement.

Compensation and Benefits

The role offers a competitive salary range of USD 120,000 to 150,000 per year, commensurate with experience.
