Work Environment

This role is based onsite at our Assisted Living Facility, where you will work alongside a multidisciplinary team, including nurses, social workers, and support staff. Key Responsibilities

• Conduct thorough health assessments and screenings, establishing baselines for both acute and chronic conditions.
• Diagnose a variety of medical conditions and develop individualized treatment plans that include preventive care and health promotion initiatives.
• Serve as a primary health care provider by managing ongoing care and facilitating referrals to specialists as needed.
• Engage with community organizations to develop and execute educational programs aimed at promoting health awareness, nutrition, and physical activity.
• Document all patient interactions and treatment details in our Electronic Health Record (EHR) system, ensuring compliance with local and federal regulations.
• Advocate for the rights of patients, providing culturally sensitive care that respects the diverse backgrounds of our community members.
• Participate in outreach programs and community health fairs to raise awareness about health resources and preventive care services available in the facility.
• Continuously update and educate yourself on the latest community health trends and best practices to improve patient care.

Patient Population / Client Focus

Your main focus will be serving the elderly population residing in our Assisted Living Facility. These individuals often face multifaceted health issues that require a compassionate and patient-centered approach. You will work to empower residents through education, promoting self-management skills, and ensuring they receive the support needed to navigate their healthcare journeys.

Qualifications

• Current certification as a Nurse Practitioner in New York State.
• Possession of an active state nursing license.
• Bachelor's degree in nursing from an accredited institution, with a strong preference for candidates holding a Master’s degree.
• Minimum of three years of clinical experience, ideally within community health or public health settings.

Preferred Experience

• Previous experience in geriatrics or working with older adults is a significant plus.
• Familiarity with community health resources and collaborative care models.
• Experience in developing and implementing health education programs tailored to specific community needs.

Compensation and Benefits
