Work Environment

As a full-time Nurse Practitioner at our Assisted Living Facility, you will work alongside a multidisciplinary team consisting of physicians, other nurse practitioners, nurses, and allied health professionals. Key Responsibilities

• Conduct thorough assessments and accurately diagnose both acute and chronic medical conditions, particularly related to pain management.
• Develop and implement individualized treatment plans, integrating evidence-based practices to improve patient outcomes.
• Order and interpret diagnostic tests, ensuring timely and appropriate responses to patient needs.
• Prescribe medications while monitoring patient responses and adjusting dosages as necessary to manage pain effectively.
• Provide patient and family education on chronic pain management strategies, lifestyle modifications, and preventative care.
• Collaborate closely with healthcare team members to coordinate comprehensive patient care, including referrals to specialists when needed.
• Maintain detailed and accurate medical records in compliance with regulatory standards and organizational policies.
• Participate in continuous improvement initiatives aimed at enhancing patient care processes and outcomes.
• Engage in professional development activities, staying informed on the latest advancements in pain management and nursing practices.

Patient Population / Client Focus

The primary patient population you will serve includes residents in our Assisted Living Facility, many of whom live with chronic pain or associated conditions. Your empathetic approach will not only address the physical aspects of pain but also consider the emotional and psychosocial factors that impact their overall wellbeing. Building strong relationships with patients and their families will be integral to your success in this role.

Qualifications

• Valid certification as a Nurse Practitioner, with a specialization in pain management or a relevant field.
• An active nursing license in New York State.
• A Master's degree in nursing from an accredited institution, demonstrating a solid foundation in advanced nursing principles.
• A minimum of 3 years of clinical experience in a healthcare setting, ideally focusing on pain management, geriatrics, or similar specialties.
• Exceptional communication and interpersonal skills, enabling effective collaboration with patients and healthcare team members.
• Strong analytical and problem-solving skills to assess patient needs and develop effective care plans.

Preferred Experience

Experience working within assisted living or similar environments is highly preferred, as is familiarity with electronic health record (EHR) systems for managing patient data. Previous involvement in quality improvement projects will be beneficial as you help drive initiatives to improve patient care and safety in our facility.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 120,000 to 130,000 , reflecting your experience and qualifications.
