Key Responsibilities\n

• Build and maintain strong therapeutic relationships with clients and their families, demonstrating reliability and trustworthiness in all interactions.

• Collaborate with a multidisciplinary team to develop and implement integrated care plans that meet clients' emotional and practical needs during their hospice experience.

• Utilize evidence-based therapeutic techniques to assess clients’ needs, provide short-term interventions, and facilitate referrals to additional support services as necessary.

• Participate actively in team meetings, contributing insights and strategies to enhance team dynamics and service delivery.

• Conduct thorough evaluations, identifying urgent needs and safety concerns, and report any potential risks in compliance with safety protocols.

Patient Population / Client Focus\n

The primary clientele consists of individuals at various stages of terminal illnesses, who may also exhibit significant mental health issues such as depression, anxiety, and grief. Your role will involve addressing these complex emotional challenges through tailored therapeutic approaches that respect individual cultural backgrounds and personal experiences. Collaboration with families and caregivers is crucial for fostering a supportive network for clients during their hospice journey.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.

• Demonstrated understanding of assessment, diagnosis, and treatment methodologies specific to severe emotional and mental health issues.

• Exceptional verbal and written communication skills that facilitate effective interactions with clients, families, and team members.
