Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' emotional and social needs, strengths, and available resources.

• Deliver individual, family, and group therapy through evidence-based modalities that may include Cognitive Behavioral Therapy (CBT), Solution-Focused Brief Therapy, and Trauma-Focused Therapy.

• Collaborate with healthcare providers, including physicians and nurses, to develop and implement holistic care plans tailored to individual patient needs.
\n
• Facilitate crisis intervention and risk assessments, providing timely support and safety planning during acute situations.
\n
• Document therapy sessions and care plans in compliance with state and federal regulations, ensuring accuracy and confidentiality.

• Lead psychoeducation workshops and support groups for families, creating a shared space for healing and shared understanding.
\n
• Utilize telehealth technology for remote therapy sessions, maintaining engagement and accessibility for clients.

Patient Population / Client Focus\n

You will primarily work with patients facing end-of-life challenges, providing psychosocial support tailored to their unique circumstances. Your expertise will help patients cope with grief, loss, and life-limiting diagnoses, while also supporting their families through the emotional complexities associated with hospice care.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• At least two years of experience in mental health care, particularly in an outpatient or hospice setting.

• Demonstrated knowledge of trauma-informed care and evidence-based therapeutic approaches.

• Exceptional interpersonal communication skills with the ability to foster trust and rapport with clients, families, and colleagues.

Compensation and Benefits\n
