Key Responsibilities\n

• Conduct comprehensive assessments for each client to accurately identify their mental health needs and tailor interventions accordingly.

• Provide ongoing communication with clients, supervisors, and family members regarding treatment progress and any changes in the care plan.

• Develop and share a tentative schedule for appointments, in-home visits, and progress observations to ensure consistent support.
\n
• Coordinate and implement follow-up home visits and interventions to support clients in their home environments, facilitating therapeutic success.

• Actively participate in Interdisciplinary Team Meetings to align on treatment goals and contribute insights reflective of your clinical expertise.

• Document treatment progress through monthly and quarterly notes, maintaining clear and consistent records that serve both client needs and organizational standards.

• Train and mentor staff in the execution of program plans and documentation processes to uphold the continuity of care.

• Establish collaborative relationships as a liaison with community stakeholders to enhance resource access and support for developmental goals.

• Contribute to the creation of training materials and procedure guidelines aimed at optimizing therapeutic programs for client benefit.

Patient Population / Client Focus\n

This position will involve working closely with a diverse array of clients, including adults and youth facing mental health challenges. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field, ensuring a strong theoretical foundation for counseling practices.

• Possess an active and unrestricted Professional Clinical Counselor (LPCC) license in the state of California, affirming your qualifications to practice.

• A minimum of two years of counseling experience in a clinical context, demonstrating proficiency in delivering mental health care.
