Work Environment

This role is based entirely onsite in the home care setting, meaning you will work directly in the homes of patients. Key Responsibilities

• Assess and evaluate patients recovering from injuries, surgeries, or chronic conditions directly in their homes.
• Develop individualized treatment plans based on comprehensive assessments, utilizing diverse therapeutic modalities, including manual therapy, tailored exercise programs, and state-of-the-art rehabilitation equipment.
• Implement treatment interventions that promote increased mobility and pain relief, regularly monitoring patient progress and adjusting therapies as required.
• Maintain thorough documentation of patient evaluations, treatment plans, and progress notes to ensure clear communication and compliance with care standards.
• Provide education and training to patients, caregivers, and families on effective use of assistive devices and best practices for rehabilitation.
• Engage in continuous learning opportunities through attending professional workshops, training sessions, and keeping abreast of the latest practice standards and research findings in physical therapy.

Patient Population / Client Focus

Our therapists serve a diverse array of patients, including individuals recovering from orthopedic surgeries, those managing chronic pain conditions, athletes rehabilitating from sports injuries, and geriatric patients who require specialized care to maintain their mobility and independence. Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• Active State license to practice as a Doctor of Physical Therapist (DPT).
• A minimum of two (2) years of clinical experience in physical therapy, ideally with a focus on orthopedic rehabilitation.
• Current CPR certification.
• Strong communication skills to effectively interact with patients and healthcare teams.

Preferred Experience

• Experience working in a home health environment and familiarity with assistive technology and devices.
• Additional certification in specialized therapeutic areas, such as manual therapy, sports rehabilitation, or geriatric physical therapy, will be viewed favorably.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience. In addition to competitive pay, we offer an extensive benefits package that includes:
