Key Responsibilities

• Conduct initial evaluations that include a thorough review of the patient's medical history and physical assessments to determine individual treatment plans.
• Clearly communicate evaluation results and treatment plans to patients and their families, involving them in goal-setting processes and ensuring their concerns and preferences are addressed.
• Implement and modify treatment protocols based on patient evaluations and progress, employing evidence-based practices to enhance recovery.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure holistic patient care and address any changes in patient condition.
• Prepare comprehensive discharge plans, ensuring continuity of care and accurate documentation of patient progress and care plans tailored to individual needs.
• Utilize effective time management skills to meet productivity benchmarks while maintaining an organized schedule and adhering to meeting commitments.
• Engage in community outreach and marketing initiatives to promote the services offered by Patient Plus Therapy and educate the public on orthopedic rehabilitation.
• Contribute positively to departmental goals and compliance standards, actively participating in quality improvement initiatives.
• Pursue opportunities for continued education and professional growth, including serving as a clinical instructor for students or mentorship roles.

Patient Population / Client Focus

The ideal candidate will work primarily with orthopedic patients, including post-operative individuals and those recovering from musculoskeletal injuries. The role requires sensitivity and adaptability to address the unique needs of diverse populations, focusing on enhancing mobility and functional independence in a comfortable home setting.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Active state licensure as a Doctor of Physical Therapy in Virginia.
• Demonstrated clinical experience with a minimum of two (2) years providing direct patient care in orthopedic settings.
• Current certification in CPR, ensuring readiness to respond to medical emergencies.

Preferred Experience

Experience in home health care, as well as knowledge of relevant software systems for managing patient records and care plans, is highly desirable. Familiarity with telehealth systems can provide additional flexibility in delivering patient care remotely, if necessary.

Compensation and Benefits
