Strongpoint Rehabilitation, LLC is actively seeking a dedicated and experienced Licensed Physical Therapist to join our team in Herndon, Virginia. This full-time, onsite position focuses on delivering exceptional care within the home care setting, allowing therapists to work directly with patients in the comfort of their own homes. Work Environment

As a Physical Therapist with Strongpoint Rehabilitation, you will work in a home care setting within the vibrant community of Herndon, Virginia. This position provides an opportunity to build meaningful relationships with patients of various backgrounds, working closely with them and their families to facilitate rehabilitation in a familiar environment. You will be expected to adhere to all local, state, and federal regulations while completing necessary training and compliance requirements, ensuring that our service standards are consistently met.

Key Responsibilities

• Conduct thorough assessments to evaluate the physical capabilities and limitations of each patient, employing evidence-based evaluation methods to inform treatment decisions.
• Design and implement individualized treatment plans based on comprehensive analysis, patient goals, and ongoing progress evaluations that adapt to each patient's evolving needs.
• Provide ongoing documentation of patient care activities, ensuring all treatments, evaluations, and progress notes are recorded accurately and in a timely manner to maintain transparency.
• Collaborate with interdisciplinary healthcare teams to ensure that patients receive holistic care, consulting with physicians and other specialists as needed to adjust treatment plans.
• Monitor patient progress through regular follow-ups, adjusting treatment strategies as necessary, and documenting any changes in condition for continuity of care.
• Participate in facility meetings and training sessions to stay current on best practices, enhancing clinical skills and fostering teamwork and collaboration among staff.
• Develop effective discharge plans to transition patients safely to their home care routines, empowering them with resources and strategies for continued rehabilitation beyond therapy sessions.

Patient Population / Client Focus

The patients you will serve include individuals experiencing a range of orthopedic issues, post-operative recovery needs, age-related mobility challenges, and other conditions requiring physical rehabilitation. Our emphasis is on patient-centered care; therefore, effective communication with patients and their families is essential to facilitate significant progress. You will play a vital role in helping patients regain independence and improve their overall physical functioning.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program is required.
• Current state licensure as a Physical Therapist (DPT) in Virginia is mandatory.
• At least two years of outpatient orthopedic experience is highly preferred, demonstrating expertise in managing various musculoskeletal conditions.
• Current certification in CPR to ensure patient safety in emergency situations.

Preferred Experience

In addition to the mandatory qualifications, experience in a home health setting and familiarity with telehealth initiatives will be advantageous. Proficiency in using electronic health record (EHR) systems for documentation and scheduling, as well as a strong understanding of patient needs assessment and treatment protocols, are valued traits for this position.

Compensation and Benefits
