Work Environment

As you join our team, you will primarily engage in patient care on-site at their residences. The flexibility of home care practice allows for personalized interactions in comforting environments. You will work Monday to Friday, totaling 40 hours a week, and will experience a dynamic and collaborative atmosphere as part of our team. The role emphasizes teamwork, communication, and mutual support among clinicians to optimize patient outcomes and experiences.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, assessing their physical abilities and rehabilitation needs.
• Develop and implement individualized treatment programs focused on orthopedic rehabilitative strategies, incorporating manual therapy and therapeutic exercises.
• Utilize evidence-based practices to guide treatment modalities that foster improvements in function and mobility.
• Educate and train patients and caregivers about procedures, exercises, and home programs to ensure continued progress outside of therapy sessions.
• Track and document patients' progress meticulously, ensuring all treatment plans align with the physician's directives and meet the organization's utilization goals.
• Uphold a high standard of customer service that enhances patients' experiences and overall satisfaction with our services.
• Collaborate with a multidisciplinary team of healthcare professionals to amplify patient outcomes through shared insights and strategies.

Patient Population / Client Focus

Your expertise will be vital in treating a diverse clientele, primarily focusing on orthopedic conditions. You will engage with patients recovering from surgeries, sports injuries, and chronic pain disorders. Our approach emphasizes personal relationships, ensuring each patient feels supported and understood throughout their recovery process.

Qualifications

• A valid Doctorate degree in Physical Therapy (DPT) is required.
• Current and valid state license to practice physical therapy in Virginia.
• Minimum of two years of professional experience in a clinical setting, particularly in orthopedic physical therapy.
• Basic Life Support (BLS) certification from the American Heart Association or equivalent.
• Exceptional verbal and written communication skills, allowing for effective patient interaction.
• Competence in using electronic health records (EHR) systems and basic word processing software.
• Demonstrated abilities in building rapport with patients, fostering trust, and motivating them to achieve their wellness goals.

Preferred Experience

• Experience in home health settings or a commitment to developing skills in this niche area of practice.

Compensation and Benefits
