Work Environment

This position is geared towards a home care setting, allowing for direct patient interaction in the comfort of their homes. The working hours are Monday through Friday, from 8 AM to 5 PM, ensuring a balanced work-life schedule. Key Responsibilities

As a Physical Therapist at Elite, your daily responsibilities will include:

• Conducting comprehensive evaluations by reviewing patient histories, performing assessments, and determining treatment recommendations tailored to individual patient needs.
• Implementing effective treatment protocols based on evidence and the initial evaluation findings, adapting these protocols as necessary to achieve optimal patient outcomes.
• Documenting patient progress accurately and consistently, including initial evaluations, daily encounter notes, interim progress summaries, and discharge evaluations to maintain continuity of care.
• Maintaining communication with other healthcare professionals, ensuring that any changes in patient conditions are recognized and addressed promptly, and collaborating on multidisciplinary care plans as needed.
• Completing all required training related to corporate and clinical services, and ensuring compliance with local, state, and federal regulations.
• Facilitating effective discharge planning, emphasizing follow-up care to support each patient's ongoing rehabilitation journey.
• Participating in team meetings and contributing to continuous improvements in patient care practices within the clinic.

Patient Population / Client Focus

You will primarily be working with patients undergoing orthopedic rehabilitation, which may include post-surgery recovery, sports injuries, arthritis management, and other musculoskeletal conditions. Our patient-centric approach ensures that all care plans are tailored to the specific challenges and goals of each individual, fostering motivation and engagement in the rehabilitation process.

Qualifications

To be considered for this position, candidates must have:

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid state license to practice as a Physical Therapist in Virginia.
• Current certification in Basic Life Support (BLS) for Healthcare Providers, as issued by the American Heart Association.

Preferred Experience

Candidates should have a minimum of two years of relevant experience in a similar clinical setting, with a strong emphasis on outpatient orthopedic care. Experience with electronic medical record systems (EMRs) and familiarity with patient management software is also highly valued.

Compensation and Benefits
