Key Responsibilities

• Deliver high-quality, safe, and compassionate care to patients in their homes, managing orthopedic recovery with personalized treatment plans.
• Implement treatment programs tailored to each patient based on established plans from supervising physical therapists, ensuring adherence to evidence-based practices.
• Conduct in-depth evaluations that include reviewing patient histories, performing specific assessments, and documenting the outcomes of treatment.
• Effectively manage discharge planning and follow-up care, including precise documentation of patient progress, evaluations, and the overall course of care.
• Maintain compliance with state, local, and federal healthcare regulations while completing requisite corporate and clinical training.
• Engage actively with co-workers, healthcare providers, and insurers to promote open communication and ensure a holistic approach to patient care.
• Participate in facility meetings and collaborative discussions to contribute ideas and insights that enhance team effectiveness.

Patient Population / Client Focus

This position primarily serves a diverse orthopedic patient population, including individuals recovering from surgeries, injuries, and chronic musculoskeletal conditions. Our focus rests on maximizing functional outcomes through patient-specific rehabilitation plans, with an emphasis on restoring mobility and quality of life.

Qualifications

• Graduate from an accredited Doctor of Physical Therapy program with a Doctorate degree in Physical Therapy (DPT).
• Possession of an active physical therapy license in Virginia, as required by the state regulations.
• Current American Heart Association BLS for Healthcare Providers CPR certification.

Preferred Experience

• A minimum of two years of hands-on experience in an orthopedic setting, providing therapy services to adult patients.

• Experience working in a home care setting is highly desirable.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to USD 120,000 annually, commensurate with experience and qualifications.
