Work Environment

This position is primarily focused on home care, meaning you will be engaging directly with patients in their own environments. You will work Monday through Friday, completing a 40-hour workweek, where each day presents a unique opportunity to make a tangible difference in your patients' lives. Key Responsibilities

• Conduct thorough evaluations and assessments of patients referred by physicians, developing and implementing personalized treatment plans.
• Utilize a range of therapeutic interventions, including therapeutic exercises, manual therapies, and patient education tailored to orthopedic, pediatric, neurologic, and sports medicine specialties.
• Maintain meticulous documentation of patient progress and treatment outcomes to ensure continuity of care and compliance with regulatory standards.
• Engage in ongoing communication with physicians, caregivers, and the interdisciplinary care team to foster collaborative treatment approaches aimed at maximizing patient outcomes.
• Educate patients and their families on rehabilitation techniques, home exercise programs, and lifestyle modifications necessary for improving mobility and function.
• Provide evidence-based care that aligns with the latest clinical guidelines, demonstrating a commitment to professional development and clinical excellence.
• Meet or exceed performance metrics related to treatment utilization, patient satisfaction, and overall business objectives.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, focusing primarily on those recovering from orthopedic injuries and surgeries. Your expertise will support individuals of all ages, including children recovering from sports-related injuries and adults undergoing post-operative rehabilitation. This specific focus allows you to apply your specialized knowledge while fostering meaningful relationships that inspire trust and encourage steady progress.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program (APTA or CAPTE).
• A valid Physical Therapy state license to practice.
• Basic Life Support (BLS) certification from the American Heart Association.
• Excellent verbal and written communication skills, with an emphasis on patient education and interpersonal relationships.
• Proficient in using electronic medical record (EMR) systems and other clinical documentation tools to streamline patient care.

Preferred Experience

While not mandatory, having prior experience in home health care or outpatient orthopedic settings will be considered a strong asset. Compensation and Benefits
