Licensed Physical Therapist - Full-Time Home Care Specialist

Position Overview

Reactive Physical Therapy is actively seeking a compassionate and skilled Licensed Physical Therapist to enhance our home care services in Vienna, Virginia. Work Environment

This position is primarily onsite in clients' homes, where you will conduct therapy sessions in a comfortable and familiar setting for our residents. This setting allows you to build strong relationships with clients and their families, understanding their specific needs and challenges. Key Responsibilities

• Conduct thorough initial evaluations and ongoing assessments of clients' physical therapy needs in their home environment to determine appropriate treatment plans.
• Develop and implement individualized treatment strategies tailored to each client's goals, focusing on enhancing physical mobility and reducing discomfort.
• Deliver evidence-based therapeutic interventions to restore function, enhance mobility, alleviate pain, and prevent disability through hands-on treatments and patient education.
• Provide education to clients and their families about exercises, mobility strategies, safety precautions, and injury prevention to foster independence at home.
• Collaborate with nursing and occupational therapy teams to ensure continuity of care and share crucial insights that inform each client's treatment plan.
• Document patient progress, clinical notes, and appropriate billing information meticulously, ensuring compliance and transparency throughout the therapeutic process.
• Engage in interdisciplinary care planning meetings and contribute to initiatives aimed at enhancing service delivery and improving patient outcomes.

Patient Population / Client Focus

Your primary focus will be on senior clients experiencing a variety of age-related physical conditions. This includes, but is not limited to, individuals recovering from surgery, managing chronic illnesses, or seeking rehabilitation following falls or other injuries. Your expertise will not only help improve their physical capabilities but also encourage emotional well-being by promoting active participation in their own care.

Qualifications

• Current and active Physical Therapist license in the state of Virginia is required.
• Bachelor's degree in Physical Therapy or a related field from an accredited institution is mandatory.
• CPR/BLS certification or the ability to obtain it upon hire.
• Strong knowledge of geriatric care protocols and experience in managing physical rehabilitation for older adults.
• Exceptional interpersonal and communication skills to effectively connect with clients and collaborate with a multidisciplinary healthcare team.
• Demonstrated empathy, patience, and a profound passion for improving the lives of older adults through therapy.

Preferred Experience

• A minimum of 2 years of experience in physical therapy, preferably in a home care or geriatric setting.
• Experience with evidence-based practices and outcome measurement frameworks tailored to individual patient needs.
• Proficient in using therapy equipment and technology to facilitate effective treatment during home visits.

Compensation and Benefits

The compensation for this role ranges from $90,000 to $100,000 annually, commensurate with experience.
