Work Environment

This role is specifically designed for home care settings, meaning that your workplace is the patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients with various functional limitations and disabilities to establish a comprehensive treatment plan tailored to individual needs.
• Provide expert consultation to other healthcare professionals regarding a patient's ongoing care, ensuring a holistic approach to health management.
• Develop and maintain effective communication with patients, their families, and interdisciplinary team members while adhering to the highest standards of customer service.
• Implement treatment plans, re-evaluate progress regularly, and educate patients and their support systems on the recovery process and best practices for success.
• Document all aspects of patient care, including evaluations, treatment plans, and billing, in compliance with regulatory standards and internal protocols.

Qualifications

Compensation and Benefits
