Work Environment

In this role, you will practice primarily onsite through our innovative home care program, allowing you to work directly in patients' homes, fostering a personalized and effective rehabilitation experience. Key Responsibilities

• Conduct comprehensive evaluations of patients in their home environment to assess their physical condition and establish individualized therapy plans.
• Implement various treatment techniques, including therapeutic exercises, manual therapy, and modalities to alleviate pain and improve mobility.
• Regularly track and document patient progress, responses to treatment, and adjust care strategies as required to ensure optimal rehabilitation outcomes.
• Educate patients and their caregivers on effective exercise regimens, safety measures, and the use of assistive devices to support self-care and independence.
• Collaborate with a multidisciplinary health team to coordinate comprehensive care that addresses all aspects of patient well-being.
• Stay current with best practices in physical therapy by attending workshops, reading relevant literature, and participating in professional organizations.

Patient Population / Client Focus

This position primarily serves adult and geriatric patients who are recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. You will work closely with individuals who may face mobility challenges and require specialized rehabilitation services to regain their functional abilities. Building a rapport with clients in a home setting allows you to better understand their unique situations and tailor your therapeutic interventions accordingly.

Qualifications

• A valid Doctorate in Physical Therapy from an accredited program is required.
• Active state license in Physical Therapy to practice in Virginia.
• Strong written and verbal communication skills to effectively interact with patients, families, and interdisciplinary teams.
• Proficient in computer skills, including documentation software and electronic health records, to maintain accurate patient records.
• Demonstrated adaptability and flexibility in response to the varying needs of clients.

Preferred Experience

• Prior experience in home health or outpatient orthopedic settings is advantageous but not mandatory.

Compensation and Benefits
