Work Environment

Your role will take place primarily in the home care setting, where you will be the face of Parkridge Adult Physical Therapy Services. Key Responsibilities

In this position, you will:

• Conduct comprehensive evaluations of patients referred by physicians, developing individualized treatment plans tailored to their specific needs.
• Utilize advanced therapeutic modalities, therapeutic exercises, and patient education strategies to improve mobility and functionality.
• Collaborate with fellow team members to optimize patient outcomes, engaging in regular case discussions and treatment planning.
• Educate patients and caregivers on rehabilitation protocols, home exercise programs, and lifestyle modifications to enhance recovery effectiveness.
• Demonstrate exceptional customer service skills, ensuring a supportive and encouraging atmosphere for each patient's rehabilitation journey.
• Adhere to documentation and administrative protocols, maintaining accurate and timely records in compliance with healthcare regulations.
• Continuously assess and adjust treatment strategies based on patient progress and feedback, ensuring alignment with the physician's care plan.

Patient Population / Client Focus

Your expertise will cater to a diverse patient population, primarily focusing on orthopedic clients recovering from surgeries, injuries, and chronic conditions. This role will also involve treating pediatric and neurologic patients, providing a well-rounded experience in various therapeutic approaches and techniques. Your capacity to tailor rehabilitation strategies to the unique circumstances of each patient will be critical in fostering recovery and enhancing their overall quality of life.

Qualifications

To be considered for this role, you must have:

• A Doctorate degree in Physical Therapy (DPT).
• A valid State License in Physical Therapy.
• A minimum of two years of professional experience in a physical therapy setting.
• A current Basic Life Support Certification from the American Heart Association.
• Strong written and verbal communication skills, enabling effective interaction with patients and their families.
• Proficient computer skills and familiarity with electronic health record systems.

Preferred Experience

While not required, experience in home health care settings, as well as previous work with orthopedic and sports medicine populations, will be viewed favorably. Familiarity with evidence-based practice guidelines and outcomes-driven therapy techniques will enhance your candidacy.
