Key Responsibilities\n

As our Licensed Clinical Social Worker, you will play a pivotal role in providing comprehensive support and intervention for our patients. Your key responsibilities will include:

• Conducting detailed clinical assessments to understand patients' emotional, psychological, and social needs, ultimately crafting personalized treatment plans that are aligned with their goals and values.

• Offering evidence-based therapeutic interventions, including but not limited to cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), ensuring that each client receives care tailored specifically to them.

• Maintaining meticulous clinical documentation in compliance with HIPAA regulations, ensuring the highest standard of patient confidentiality and care quality.

• Continuously engaging in collaborative treatment planning with an interdisciplinary team, including medical professionals and other social workers, to provide holistic and coordinated care.

• Participating in case reviews and professional development activities, ensuring your skills remain sharp and up-to-date with best practices in hospice care.

• Empowering patients and families by educating them about mental health issues, available resources, and necessary interventions, fostering resilience and understanding.

Patient Population / Client Focus\n

The primary focus of your role will be on patients receiving hospice care, as well as their families. You will work with individuals facing terminal illnesses, providing them with support that considers their unique cultural, emotional, and social contexts. Your work will be aimed at improving the quality of life during their remaining time, addressing psychological distress and enhancing family dynamics.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two years of post-licensure experience providing clinical social work services, preferably in hospice or palliative care settings.
