Key Responsibilities\n

• Collaborate with primary care providers to identify and address patients' mental health and psychosocial issues, ensuring comprehensive care.

• Conduct detailed patient intakes that focus on diagnostic assessments and functional evaluations, subsequently recommending tailored treatment plans to primary care teams.

• Provide brief, focused therapy interventions for patients requiring mental health services.
\n
• Evaluate which patients may benefit from continued treatment in a primary care setting versus those needing referral to specialized mental health facilities.

• Initiate proactive follow-ups to gauge patient progress and adjust treatment plans as necessary.

• Develop and implement relapse prevention strategies while aiding patients in maintaining stable functioning.
\n
• Identify at-risk patients and develop intervention plans aimed at preventing deterioration of their mental health.
\n
• Facilitate continuity of care by coordinating behavioral health services and liaising with other treatment providers, while maintaining patient confidentiality.

• Maintain accurate documentation of patients' progress and keep primary care providers informed of any changes in treatment requirements.

• Empower patients by providing essential self-management skills and educational resources, encouraging full participation in their treatment journey.

• Consult with clinic management and fellow team members about improving behavioral services and evaluating outcomes.

• Assist patients in navigating their medical treatments, including strategies to manage any medication side effects.

• Adhere to the highest standards of ethical practice, maintaining compliance with the Code of Ethical Conduct for Clinical Professionals.

• Carry out other related duties as assigned to support clinic operations.

Patient Population / Client Focus\n

The LPC will primarily serve a diverse client base, addressing various mental health conditions across age groups. Qualifications\n

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
