Work Environment

This position is uniquely set within the home care environment, where our therapists visit patients in their residences. Our standard working hours are from 9 am to 5 pm, Monday through Friday, allowing for a robust work-life balance.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical capabilities and limitations to develop individualized care plans that address their unique rehabilitation goals.
• Provide targeted therapeutic interventions aimed at restoring motion, mobility, and overall functionality using evidence-based techniques.
• Maintain a manageable patient load to ensure high-quality care and offer sufficient time for each patient's unique needs.
• Utilize effective, non-invasive treatment methods that reduce the need for medication and surgical intervention.
• Educate patients on exercise techniques and wellness strategies that promote recovery and prevent future injuries.
• Implement fitness and wellness programs customized to each patient's needs, fostering their participation in their own rehabilitation journey.
• Collaborate with a multidisciplinary team to leverage collective expertise in optimizing patient care and treatment effectiveness.
• Document patient progress accurately in an electronic medical record (EMR) system, specifically Web PT, ensuring care continuity and compliance with healthcare standards.
• Regularly evaluate treatment outcomes and adjust care plans as needed to meet evolving patient goals.

Patient Population / Client Focus

This role primarily services a diverse patient population, including individuals recovering from orthopedic surgeries, sports injuries, chronic pain conditions, and high-risk patients requiring specialized attention. By working directly in patients' homes, you will gain unique insights into their daily lives, allowing you to provide personalized therapeutic strategies and education regarding physical activity and wellness.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• An active Physical Therapist license in the state of New Jersey, free from sanctions or any history of board proceedings.
• Exceptional proficiency in a variety of manual therapy techniques tailored to patient needs.
• Strong experience in developing and executing effective exercise programs that yield positive outcomes.
• Proficiency in using EMR software, particularly Web PT, ensuring precise documentation and patient tracking.
• A commitment to staying informed about the latest evidence-based practices in physical therapy.

Preferred Experience

• Previous experience working in home health care settings is preferred.
• Familiarity with orthopedic outpatient care settings, preferably with a sports medicine background.

Compensation and Benefits
