Key Responsibilities\n

• Conduct thorough virtual psychosocial assessments, utilizing established frameworks to identify patients’ strengths, needs, and treatment goals.

• Deliver evidence-based interventions through individual, family, and group therapy sessions for clients navigating the complexities of mental health challenges.

• Work alongside healthcare providers—including nurses, medical doctors, and care coordinators—to ensure an integrated approach to client wellness, focusing on both mental and physical health needs.

• Advocate for clients’ rights, helping them access community resources such as support groups, financial assistance, and other essential services that enhance their quality of life.

• Maintain meticulous documentation of client interactions, treatment plans, and progress in our electronic health record system to ensure continuity of care and compliance with regulations.

• Actively participate in multidisciplinary team meetings and case conferences, contributing insights and feedback that will help refine treatment strategies and improve client outcomes.

Patient Population / Client Focus\n

Your work will primarily focus on patients in a hospice care setting, many of whom face terminal illnesses and complex psychological distress. By utilizing your social work expertise, you will address not only their emotional and psychological needs but also provide support to their families during these challenging times. Your role is critical in facilitating conversations around end-of-life decisions, grief, and coping strategies, thereby enhancing the experience for both clients and their loved ones.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• A minimum of two years of post-licensure clinical experience in social work, with a focus on mental health services.

• Proven proficiency in conducting diagnostic assessments and developing individualized treatment plans that are adaptive to clients' needs.

Preferred Experience\n
