Key Responsibilities\n

• Conduct virtual psychosocial assessments to evaluate clients' strengths, challenges, and support networks.

• Provide evidence-based therapeutic interventions, including individual and group therapy, to address a spectrum of mental health issues.

• Engage with interdisciplinary teams comprised of healthcare providers, physicians, and hospice staff to coordinate integrated care for clients.

• Meticulously document client interactions, treatment plans, and progress in electronic health records to ensure continuity of care.

• Participate in team meetings and case conferences to discuss treatment goals and progress, and to share insights on best practices in social work.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• At least two (2) years of post-licensure clinical experience, specializing in mental health services.
