Key Responsibilities\n

• Communicate effectively with physicians and healthcare teams to report on the progress of patients, identifying needs, concerns, and any potential problems that may arise during treatment.

• Maintain accurate and timely records of clinical assessments, treatment plans, and progress notes following department policies and best practices in healthcare documentation.

• Work collaboratively with multidisciplinary teams to develop individualized treatment plans, including discharge plans based on comprehensive psychosocial assessments and evidence-based practices.

• Provide counseling and crisis intervention to patients experiencing emotional and behavioral challenges, tailoring your approach to meet their unique needs.

• Assist clients with discharge planning, ensuring they have a clear understanding of their next steps, and conduct care coordination with relevant providers within our system.
