Key Responsibilities\n

• Conduct thorough ongoing assessments of patients to identify their mental health needs and inform treatment planning.

• Maintain open communication with supervisors, direct care staff, and families to report on patient progress, ensuring a holistic approach to care.

• Develop and provide a tentative schedule of appointments and home visits to effectively monitor patient progress and support services.

• Coordinate home visits to conduct in-home assessments, perform chart reviews, and gather data through staff interviews and observations.

• Ensure that the implementation of treatment plans is effective and adheres to the established program protocols.

• Engage actively in Interdisciplinary Team Meetings to discuss patients' cases and collaborate on comprehensive care strategies.

• Document patient progress through monthly and quarterly progress notes to track improvements and setbacks accurately.

• Organize training and workshops to enhance the competency of team members on program protocols and documentation standards.
\n
• Provide continual support through follow-up interviews and home visits, addressing any evolving needs of the patients.

• Maintain comprehensive records, ensuring all necessary information is accessible to authorized parties.

• Serve as a liaison between the clinic and the public, fostering relationships to enhance community mental health resources and service awareness.

Qualifications\n
