Key Responsibilities

• Patient Assessment: Conduct comprehensive evaluations to assess the physical, functional, and rehabilitative status of each patient, using techniques in accordance with professional standards and regulatory guidelines.
• Care Planning: Develop individualized care plans in collaboration with the patient and healthcare team, outlining specific goals and interventions tailored to achieve optimal functional outcomes.
• Therapeutic Interventions: Deliver evidence-based physical therapy treatments and modalities, including manual therapy, therapeutic exercise, gait training, and education on injury prevention.
• Patient and Family Education: Instruct patients and their families on exercises, therapeutic appliances, and home modification strategies to support ongoing rehabilitation beyond formal therapy sessions.
• Coordination: Coordinate effectively with other members of the patient care team, including physicians, nurses, and caregivers, to ensure seamless continuity of care.
• Documentation: Maintain accurate, detailed documentation of patient progress, treatment plans, and outcomes in compliance with organizational policies and healthcare regulations.

Patient Population / Client Focus

As a Physical Therapist at Vitalize Physical Therapy, you will work primarily with adult patients recovering from surgeries, injuries, or managing chronic conditions. Qualifications

• Current California license to practice as a Physical Therapist.
• Bachelor's degree in Physical Therapy from an accredited program.
• Basic Cardiac Life Support (BCLS) certification or instructor status should be obtained within 30 days of hire.

• While no prior experience is required, any hands-on clinical training or internships in a home care setting will be advantageous.
• Experience working with varied patient demographics, including pediatrics, geriatrics, or post-surgical rehabilitation is a plus.

Compensation and Benefits
