In this full-time role, you will deliver comprehensive physical therapy services in a home care setting, aiming to enhance the quality of life for our patients through targeted rehabilitation.

Work Environment

This position is onsite and primarily involves visiting patients in their homes, which allows for personalized care tailored specifically to each individual's environment. As part of our interdisciplinary team, you will collaborate with other healthcare providers to deliver coordinated services that address both physical and emotional wellness. The work schedule is Monday through Friday from 9 am to 5 pm, with flexibility to accommodate patient needs.

Key Responsibilities

• Conduct thorough evaluations of patients to accurately assess their physical conditions, functional capabilities, and rehabilitation needs.
• Design and implement individualized treatment plans based on evaluation results, ensuring alignment with each patient's specific health goals.
• Utilize a variety of hands-on techniques, therapeutic exercises, and modalities such as electrical stimulation, heat therapy, and ice application to facilitate recovery and improve patient mobility.
• Educate patients and their families on effective body mechanics, proper posture, and preventative measures to minimize the risk of injury.
• Regularly monitor patient progress, adjusting treatment plans as necessary to optimize outcomes and address evolving health conditions.
• Collaborate with physicians and other healthcare practitioners to ensure cohesive and holistic care for each patient.
• Maintain meticulous and accurate documentation of patient interactions, interventions, and outcomes in accordance with healthcare regulations.

Patient Population / Client Focus

Our patient population encompasses a diverse demographic, including individuals recovering from surgery, those experiencing chronic pain from conditions such as arthritis, and patients needing rehabilitation for sports-related injuries. We also focus on geriatric care, helping seniors improve their mobility, strength, and overall quality of life through personalized interventions.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Current and valid state licensure to practice as a Physical Therapist.
• Board certification from the American Physical Therapy Association (APTA) is strongly preferred.
• A minimum of one year of clinical experience in physical therapy is required; five years of experience is preferred.
• Proficiency in advanced therapeutic techniques and modalities related to rehabilitation and patient care.
• Strong analytical abilities to assess patient needs and determine optimal care strategies.
• Excellent communication skills, with a focus on building rapport with patients, families, and the healthcare team.

Preferred Experience

Ideal candidates will have experience in a home care setting, providing therapy in a patients' residence, which enables a tailored approach to rehabilitation. Familiarity with electronic health records (EHR) systems is advantageous for maintaining accurate patient documentation.

Compensation and Benefits
