Vitality Care Clinic is seeking a dedicated and licensed Physical Therapist (PT) to join our team full-time, providing exceptional therapeutic services in Chula Vista, California. Work Environment

This full-time position is onsite, primarily focused on home care, allowing therapists to work closely with clients in a familiar environment. Your work week will typically run from Monday to Friday from 8 AM to 3 PM, providing an opportunity to balance your professional and personal life effectively.

Key Responsibilities

• Conduct thorough Physical Therapy evaluations for clients, identifying their strengths and areas for improvement.
• Design and implement individualized treatment plans that target specific goals, using evidence-based practices tailored to each patient's condition.
• Regularly reassess patient conditions, adjusting treatment plans as needed to ensure progress toward therapeutic goals.
• Maintain accurate and timely documentation, including SOAP notes and progress summaries, using our electronic health record system.
• Educate patients and their families about treatment protocols, recovery strategies, and potential challenges, fostering their active participation in the healing process.
• Achieve minimum utilization goals and meet other business objectives while ensuring high-quality patient care.
• Work collaboratively with interdisciplinary teams, leveraging collective expertise for clients receiving multiple services.

Patient Population / Client Focus

You will work with a diverse patient base, including children and adults of all ages and backgrounds. The spectrum of conditions may include orthopedic injuries, neurological disorders, rehabilitation post-surgery, developmental delays, and more. Your ability to adapt therapy techniques to suit different age groups and cultural contexts will be crucial for effective treatment and engagement.

Qualifications

• A Master's or Doctor of Physical Therapy (DPT) degree from an accredited program is required.
• Current and unrestricted licensure from the California Board of Physical Therapy.
• Willingness to deliver evaluations and treatment both in-person and via telehealth when clinically appropriate.
• A genuine passion for working with a wide range of patients, exhibiting cultural competence and empathy.
• Flexibility to adapt to dynamic work settings and changing patient needs.
• Strong collaborative spirit and commitment to customer service.

Preferred Experience

• Experience in home care settings and knowledge of telehealth best practices.

Compensation and Benefits
