Key Responsibilities\n

• Conduct ongoing assessments to identify and address the mental health needs of patients, ensuring comprehensive care tailored to individual requirements.

• Facilitate open communication with supervisors, direct care staff, family members, and guardians to monitor and discuss each client's progress.

• Prepare and share appointment schedules, home visit plans, and observation protocols for effective tracking of patient engagement and development.

• Coordinate home visits as necessary to conduct in-home observations, complete chart reviews, and perform staff interviews, ensuring a holistic understanding of each client’s situation.

• Oversee the successful implementation of individual program plans, ensuring strategies are enacted and monitored effectively.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing valuable insights and collaborative approaches to client care.
\n
• Document monthly and quarterly progress notes accurately to reflect ongoing patient development and necessary adjustments to care plans.
\n
• Organize and facilitate training sessions for team members, ensuring adherence to best practices in program implementation and documentation.
\n
• Engage in follow-up interviews and additional home visits to provide continued support to clients, enabling sustained progress toward their mental health goals.

• Act as a liaison with the mental health community to support clients’ developmental goals, fostering positive relationships that enhance community resources.

Patient Population / Client Focus\n

You will work with a diverse client base, including both adults and youth, who are experiencing a range of mental health challenges. This diversity in the patient population allows for rich clinical experiences and the opportunity to develop specialized treatment approaches tailored to various issues such as anxiety, depression, trauma, and substance misuse.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted Licensed Professional Counselor (LPC) credential in the state of New Jersey is mandatory.

• A minimum of two years of experience in clinical counseling is necessary, preferably within an outpatient setting.

• A specialization in family therapy, couples therapy, substance misuse, or trauma is highly preferred.

• Strong understanding of legal and ethical considerations in mental health practice.
