Key Responsibilities\n

• Engage in effective communication with referring physicians, sharing updates on patient progress, needs, concerns, and any potential issues that may arise.

• Maintain comprehensive and accurate clinical documentation, including progress notes and treatment plans, in compliance with departmental policies and legal requirements.

• Develop and implement treatment plans based on psychosocial assessments, incorporating evidence-based interventions tailored to each client's unique circumstances.

• Provide counseling and crisis intervention services, focusing on the assessment and management of emotional and behavioral problems that may impact clients’ lives.
\n
• Assist clients in developing personalized discharge plans, coordinating care across multiple providers and support systems to ensure a smooth transition and continued support.

• Facilitate family consultations and engage support systems in the treatment and discharge planning process, working collaboratively to address client needs.

Patient Population / Client Focus\n

You will primarily work with adults in an outpatient context who are navigating a variety of mental health challenges, including anxiety disorders, depression, trauma, and relationship issues. Our clients come from diverse backgrounds, and as such, you will apply culturally competent practices to meet their individual needs, ensuring all clients feel understood and valued.

Qualifications\n

• A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology is required.
\n
• Current licensure as an LMFT, LPC, LCSW, or equivalent credential approved by the state of Pennsylvania.

• A minimum of three years of experience providing mental health services in an outpatient setting, with demonstrated proficiency in therapeutic techniques.

• Demonstrated ability to work independently, manage a caseload effectively, and maintain a high level of organization and self-direction.

• Strong computer literacy, particularly with MS Office Suite, as well as experience with electronic health record (EHR) systems.

Preferred Experience\n

• Experience in delivering evidence-based treatment modalities, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT).

• Prior involvement in interdisciplinary team settings and familiarity with care coordination processes.

• Knowledge of community-based resources and services to facilitate patient access and support.
