Key Responsibilities\n

• Conduct thorough psychosocial assessments including interviews, observations, and analysis of medical records to identify emotional, social, and environmental factors affecting recovery.

• Provide one-on-one counseling and support for patients and their families, particularly concerning adjustment issues related to their health status and improving emotional well-being.

• Collaborate with a multidisciplinary rehabilitation team to devise individualized treatment plans that address both clinical and psychosocial needs of the patients.

• Assist patients in navigating community resources—such as support groups, vocational rehabilitation, and housing assistance—to enhance their post-rehabilitation support systems.

• Educate patients and their families on available community resources and aftercare services, facilitating a smooth transition from active rehabilitation to independent living.

• Engage in discharge planning to ensure that the needs of the patient are met and that they have a solid plan in place for ongoing support.

Patient Population / Client Focus\n

Our patient demographic is diverse, encompassing individuals recovering from conditions like stroke, spinal cord injury, orthopedic surgeries, and neurological disorders. You will address a spectrum of psychosocial needs, focusing on supporting vulnerable populations in their emotional and social readiness for reintegration into the community.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is mandatory.

• Current active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
