Work Environment

This position is primarily onsite, allowing you to engage directly with clients in their home environments. As a Full-Time Home Care Physical Therapist, you will operate in diverse residential settings, making each day unique while building strong relationships with patients and their families. You will work Monday through Friday, contributing to a schedule that supports work-life balance while surrounding yourself with a collaborative team of healthcare professionals who share a common goal of delivering outstanding patient care.

Key Responsibilities

• Direct physical therapy treatments tailored to prevent disability, alleviate pain, and promote muscle function recovery in alignment with established care plans.
• Supervise and support physical therapy assistants to ensure fidelity in treatment implementation as directed in patient care plans.
• Perform thorough evaluations of clients' functional abilities using a variety of diagnostic tests to guide treatment decisions.
• Assess home environments to identify barriers to rehabilitation, providing practical recommendations that enhance client independence.
• Deliver prescribed physical therapy services, including therapeutic exercises, manual therapies, and use of physical agents such as traction and massage.
• Fit and adjust prosthetic and orthotic devices as required, communicating necessary modifications to physicians for optimal patient benefit.
• Contribute to the development and revision of client care plans, ensuring they remain current and relevant to changing patient needs.
• Document clinical records, including evaluations, progress notes, and discharge summaries, maintaining comprehensive and accurate patient files.
• Collaborate with interdisciplinary teams to facilitate case discussions and share insights regarding client treatment and progress.
• Provide education and training for clients and their families on physical therapy regimens and assistive devices, promoting ongoing participation in care.
• Demonstrate accessibility and guidance to physical therapy assistants, ensuring adherence to practice standards and state regulations.
• Complete additional tasks as assigned to fulfill the organizational mission and improve patient outcomes.

Patient Population / Client Focus

You will engage with a diverse patient population ranging from post-operative recovery clients to individuals with chronic illnesses. Your work will directly influence their rehabilitation journeys and overall quality of life, as you implement evidence-based approaches that support their physical capabilities within the home care setting.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Virginia state Physical Therapy license in good standing.
• Current CPR certification demonstrating your readiness for emergency response.
• A year of relevant experience as a practicing physical therapist, preferably in acute care or home care settings.
• Strong interpersonal and communication skills, enabling effective engagement with patients and their families.

Compensation and Benefits

This position offers a competitive annual salary range between USD 100,000 and USD 120,000, commensurate with experience and qualifications.
