Work Environment

This role is set in a nurturing home care environment, where you'll interact closely with patients in their daily living settings. You will need to adapt your therapeutic techniques based on the diverse home environments and individual health needs of clients. Key Responsibilities

• Direct, implement, and evaluate physical therapy treatments that focus on preventing disabilities, relieving pain, and restoring mobility based on the patient's specific needs.
• Supervise and mentor Physical Therapy Assistants in delivering care as outlined in the patient's plan of care, ensuring adherence to best practices.
• Conduct comprehensive assessments using diagnostic tests to measure muscle, nerve, and joint functions, and adjust treatment plans accordingly.
• Evaluate home environments for rehabilitation efficacy, recommending necessary adjustments to enhance the patient's independence.
• Administer therapeutic exercises and functional activities, guiding clients through variations of manual and non-manual therapies.
• Document clinical records meticulously, including evaluations, progress notes, and treatment summaries compliant with OASIS and other regulations.
• Regularly review and adjust care plans, actively communicating with physicians and case managers about changes in the patient's condition.
• Participate in interdisciplinary team meetings, contributing insights from your evaluations to foster a holistic approach to patient care.
• Engage with patients and families, providing education about therapy techniques, assistive devices, and overall health maintenance.
• Manage administrative tasks, including timely documentation for clinical and payroll purposes, aligned with branch policy.
• Capably handle additional responsibilities and assignments to meet the needs of the organization and its goals.

Patient Population / Client Focus

Your role will primarily focus on patients requiring home care services, including elderly clients, those recovering from surgery, and individuals managing chronic illnesses. You will develop close relationships with your patients, enabling more personalized care and fostering trust, essential for successful rehabilitation. Your therapeutic interventions can significantly impact their quality of life, guiding them towards greater independence and improved health outcomes.

Qualifications

• Must hold a Doctorate in Physical Therapy (DPT) from an accredited institution.
• One year of experience as a physical therapist is required, preferably in either acute care or home care settings.
• Current licensure as a Physical Therapist in Virginia, maintaining good standing.
• Current CPR certification to ensure patient safety in various situations.
• Strong interpersonal and communication skills, with a focus on family engagement and teamwork.
• Commitment to clinical excellence, embracing a patient-centered approach at all times.
• Ability to navigate federal and state healthcare regulations competently.

Preferred Experience

While the position requires a minimum one year of experience, candidates with experience in innovative therapy approaches or additional certifications in gerontology or home health services will be highly regarded. Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
