Work Environment

This position will primarily involve onsite work within the homes of patients throughout the Centreville area. You will work closely with clients in a one-on-one setting, enabling a tailored approach to their rehabilitation needs. Key Responsibilities

As a Physical Therapist at PerformAX, your responsibilities will include:

• Designing and implementing effective physical therapy treatment plans aimed at relieving pain, restoring muscle function, and enhancing overall mobility in accordance with physician prescriptions.
• Evaluating the client's level of physical function and independence through thorough assessments, including diagnostic and prognostic testing of muscle, nerve, joint, and functional capabilities.
• Overseeing Physical Therapy Assistants to ensure the safe and correct application of established care plans.
• Conducting in-home evaluations of the environment and making recommendations to enhance the client's ability to perform daily activities independently.
• Administering therapeutic exercises, manual therapies, and physical modalities such as traction and massage as indicated by the treatment plan.
• Completing comprehensive clinical documentation for each patient, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring adherence to both legal standards and company procedures.
• Participating actively in interdisciplinary team meetings to collaborate on care plans and patient objectives.
• Training and guiding clients and their families on the correct use of therapy equipment and exercises necessary for recovery.
• Maintaining up-to-date knowledge of federal, state, and agency regulations to ensure compliance in all practices.
• Engaging in quality improvement initiatives to enhance service delivery and patient outcomes.

Patient Population / Client Focus

You will work primarily with a diverse patient population that includes individuals recovering from surgery, those with chronic pain conditions, and patients with various disabilities. Your role will be crucial in helping these clients regain independence and improve their quality of life through tailored rehabilitation strategies.

Qualifications

To be considered for this position, candidates must meet the following qualifications:

• Graduation from an accredited Doctor of Physical Therapy (DPT) program.
• A current and valid physical therapy license in Virginia, in good standing.
• A minimum of one year of professional experience as a physical therapist, preferably in home care or acute care settings.
• Current CPR certification.
• Strong commitment to the principles of patient-centered care and clinical excellence.
• Exceptional interpersonal communication skills and a collaborative mindset.
• Ability to adapt and thrive in a dynamic home healthcare environment.

Preferred Experience

While not mandatory, candidates with prior experience in home health care settings or those familiar with advanced therapeutic techniques and modalities will be highly regarded. Experience in a multidisciplinary team approach is also advantageous.

Compensation and Benefits

PerformAX offers a competitive salary ranging from USD 100,000 to USD 120,000 annually, commensurate with experience and qualifications.
