Work Environment

This role will be performed onsite, specifically in our clients' homes throughout the beautiful city of San Francisco. The position provides a nurturing and dynamic work environment, where you will not only engage directly with clients but also collaborate with family members and other healthcare professionals. The flexibility of our scheduling, from Monday to Friday, allows you to balance a demanding workload while maintaining personal time.

Key Responsibilities

• Direct physical therapy interventions that aim to prevent disabilities, relieve pain, and improve or restore muscle function, ensuring that maximum performance is maintained according to each client's care plan.
• Supervise and guide Physical Therapy Assistants to ensure the proper implementation of therapy protocols and adherence to care plans.
• Conduct thorough evaluations of clients' physical abilities utilizing diagnostic tests to establish a baseline and create personalized rehabilitation goals.
• Assess home environments for rehabilitation needs and recommend necessary modifications to enhance client independence.
• Execute a variety of treatments including manual and non-manual therapeutic exercises, traction, massage, and other physical modalities, as prescribed by physicians.
• Evaluate, fit, and adjust prosthetic and orthotic devices, providing feedback and recommendations to the physician.
• Participate actively in care plan development, monthly reviews, and interdisciplinary meetings to ensure comprehensive client care.
• Accurately complete clinical documentation including evaluations, progress notes, and discharge summaries while adhering to OASIS requirements.
• Educate clients and their families on physical therapy programs and the proper use and care of therapeutic devices.
• Be an accessible resource for Physical Therapy Assistants, providing guidance as necessary during treatment sessions.
• Complete additional assignments as required, contributing to overall company objectives and goals.

Patient Population / Client Focus

You will primarily work with adult and geriatric populations who may face a range of physical challenges such as recovering from post-surgical procedures, managing chronic conditions, or regaining mobility after injury. Your expertise will significantly impact their rehabilitation journey, helping to restore mobility and enhance their overall quality of life.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• Active physical therapy licensure in California with a clean record.
• Current CPR certification and a valid driver's license with automobile insurance.
• A passion for clinical excellence and a commitment to delivering high-quality patient care.
• Strong interpersonal skills and the ability to effectively communicate with patients, families, and multidisciplinary team members.

Preferred Experience

• A minimum of one year's experience as a Physical Therapist, with experience in either acute care or a home care setting preferred.
• Demonstrated ability to collaborate in an interdisciplinary team environment and engage in patient-centered care practices.
• A proactive approach to problem-solving and a willingness to adjust therapeutic techniques based on client responsiveness.

Compensation and Benefits

The salary range for this position is between USD 100,000 and 120,000 per year, commensurate with experience and qualifications.
