Work Environment

This position is primarily onsite, as you will be visiting patients' homes in and around San Francisco. Working in diverse environments, you will adapt your therapy practices to fit each patient's unique living situation. Your schedule will typically be Monday to Friday, from 9 AM to 5 PM, providing you with a structured yet flexible work-life balance, aligned with our commitment to employee well-being.

Key Responsibilities

• As a Home Care Physical Therapist, you will evaluate and treat patients recovering from orthopedic conditions, injuries, or surgeries, ensuring that each patient's specific needs are met through personalized rehabilitation strategies.
• Develop tailored treatment plans based on assessments, utilizing techniques such as therapeutic exercises, manual therapy, and mobility training to improve patient function and reduce pain.
• Document all treatments, progress, and patient responses meticulously to maintain accurate treatment records that facilitate continuity of care.
• Regularly assess therapeutic progress by re-evaluating treatment goals and adjusting therapy techniques accordingly to align with patient recovery stages.
• Instruct and support patients and their families in the use of assistive devices and self-care strategies vital for independent living.
• Engage with caregivers and interprofessional healthcare teams to provide holistic patient education and to integrate physical therapy goals with overall health plans.
• Continuously enhance your professional knowledge by attending workshops, engaging with literature, and contributing to professional communities.

Patient Population / Client Focus

Your work will encompass a wide array of patients, from post-operative orthopedic patients needing rehabilitation to elderly clients requiring support in mobility and daily activities. Qualifications

• A minimum of a Doctorate in Physical Therapy (DPT) is required, alongside a valid State License to practice Physical Therapy in California.
• At least two years of professional experience in physical therapy, particularly in an orthopedic setting, is preferred, showcasing your ability to assess and treat musculoskeletal disorders effectively.
• Excellent communication skills are essential for interacting with patients, caregivers, and interdisciplinary teams, ensuring clear understanding of treatment recommendations.
• Proficiency in computer skills, including word processing and electronic medical records, will be necessary for documentation and reporting purposes.

Preferred Experience

• Experience in home health care or community health services is a plus, enabling you to navigate the unique challenges of providing care in a patient's home.

Compensation and Benefits

The salary range for this position is competitive, between USD 90,000 and 110,000 annually, supplemented by a comprehensive benefits package that includes medical, dental, and vision insurance.
