Key Responsibilities\n

• Utilize your clinical expertise to independently formulate psychosocial diagnoses based on comprehensive assessments of patients.

• Collaborate with patients to create tailored, patient-centered treatment plans that address their unique needs and circumstances.

• Implement evidence-based therapeutic interventions, employing various techniques such as solution-focused treatment, behavioral activation, motivational interviewing, and more to enhance patient outcomes.

• Coordinate effectively with multidisciplinary teams, including psychologists, primary care providers, and external service providers, to ensure a comprehensive and integrated approach to care.

• Monitor patient progress systematically through regular follow-ups, either in person or via telehealth platforms, ensuring adherence to treatment goals.

• Provide support regarding prescribed psychotropic medication management, including monitoring treatment adherence, identifying side effects, and assessing overall efficacy.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Possession of a current, valid, and unrestricted license to practice independently in mental health as a Licensed Clinical Social Worker (LCSW) within your state.
