Key Responsibilities\n

• Conduct comprehensive assessments to identify the unique mental health needs of each patient, ensuring they receive individualized treatment.

• Maintain effective communication with supervisors, colleagues, and the families of clients to keep everyone informed about patient progress and to coordinate care effectively.

• Create and manage a meticulous schedule of appointments, home visits, and therapy sessions that align with the needs of your caseload.

• Coordinate and carry out in-home observations and visits, conducting thorough chart reviews to ensure an accurate understanding of patient situations.

• Monitor and implement program plans to evaluate their effectiveness, making necessary adjustments based on patient feedback and clinical observations.

• Participate in interdisciplinary team meetings, contributing valuable insights and collaborating on patient care strategies.
\n
• Document patient progress monthly and quarterly through detailed reports that track therapeutic progress and outcomes.
\n
• Oversee training sessions for team members, ensuring that best practices in program implementation and documentation are effectively taught.
\n
• Provide ongoing follow-up through interviews and visits to sustain support for patients after treatment.

• Act as a liaison with the public and other mental health organizations to advocate for patient development goals and resources.

• Create and improve training materials to enhance program delivery and patient engagement.

Patient Population / Client Focus\n

This role involves working with a diverse client base, including adults and youth facing a variety of mental health challenges, such as anxiety, depression, trauma, and substance use disorders. You will have the opportunity to apply specialized methodologies in family therapy, couples therapy, or other evidence-based practices to enhance therapeutic relationships and facilitate positive outcomes.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee.

• A minimum of 2 years of counseling experience, ideally in a clinical setting.

• Strong understanding of the ethical and legal standards relevant to mental health practice.

• Exceptional interpersonal and communication skills, demonstrating the ability to establish rapport with diverse client populations.

• Demonstrated capability to work collaboratively within a healthcare team.

• Proficiency in electronic medical record (EMR) software and related technologies essential for documentation and patient management.

Preferred Experience\n

Compensation and Benefits\n

The annual salary for this position ranges from $90,000 to $100,000 based on experience and availability.
