Key Responsibilities\n

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), or Licensed Marriage and Family Therapist (LMFT) in Illinois.

• Demonstrated proficiency in providing therapeutic services to clients with integrated care needs, preferably those with co-occurring medical conditions.
