Key Responsibilities\n

• Facilitate individual and group therapy sessions through secure video conferencing platforms, employing evidence-based therapeutic techniques tailored to various client needs.

• Collaboratively develop, implement, and regularly assess individualized treatment plans that align with the specific goals and preferences of clients.

• Engage in interdisciplinary collaboration by teaming up with medical professionals, nursing staff, and caregivers to ensure comprehensive patient care and support.

• Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress notes, ensuring compliance with all relevant regulatory and ethical standards.

Patient Population / Client Focus\n

In this role, you will primarily support clients navigating end-of-life issues or chronic illnesses, along with their families, offering empathetic guidance and coping strategies during difficult transitions. Qualifications\n

• Possession of a valid Licensed Clinical Social Worker (LCSW) license in New York.

• A Bachelor's Degree in Social Work or a related field from an accredited institution.

Compensation and Benefits\n

This rewarding role offers a competitive salary ranging from USD 115,000 to 130,000 per year , commensurate with experience and qualifications.
