Key Responsibilities\n

• Conduct thorough psychosocial assessments aimed at identifying the strengths and needs of patients and their families, focusing on effective coping mechanisms during hospice care.

• Deliver evidence-based therapy and counseling services, both individually and in groups, to support emotional well-being, resilience, and adaptive coping strategies.

• Engage with a multidisciplinary team of healthcare providers, including nurses, physicians, and spiritual care specialists, to coordinate comprehensive patient care that meets all aspects of well-being.

• Advocate for patients' rights and facilitate access to essential community resources, enhancing the overall support network for patients and their families.

• Maintain accurate and detailed documentation of client interactions, treatment plans, progress, and outcomes in our electronic health records to ensure continuity of care.

Patient Population / Client Focus\n

This role primarily focuses on individuals receiving hospice care, requiring sensitivity and expertise in handling difficult conversations regarding end-of-life issues, grief, and loss. As a social worker within this context, you will engage with a diverse range of patients, including those from various cultural backgrounds, and their families, helping them navigate emotional uncertainties while fostering resilience.

Qualifications\n

• A Master's degree in social work from an accredited program is essential.

• Active and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• A minimum of two (2) years of post-licensure experience providing clinical social work services, ideally within a mental health or hospice setting.

• Demonstrated competence in conducting comprehensive diagnostic assessments and creating effective treatment plans tailored to unique patient needs.
