Key Responsibilities\n

• Documentation and Reporting: You will prepare and maintain electronic medical records in compliance with departmental and health system policies, ensuring that all data is accurately recorded and up-to-date.

• Assessment: Conduct thorough psychosocial assessments of patients and their families upon admission and at designated intervals to identify and evaluate their social, emotional, and physical needs.

• Treatment Planning: Collaborate extensively with interdisciplinary healthcare teams to develop effective psychosocial treatment plans tailored to the needs of patients and their families.

• Support and Counseling: Provide supportive counseling, crisis intervention, and therapeutic services that empower patients and families to navigate the challenges associated with illness and healthcare changes.

• Caseload Management: Independently manage a diverse caseload that varies in clinical acuity, employing effective assessment and intervention strategies.

• Community Resource Coordination: Coordinate referrals to specialty services and community entitlements to equip patients and families with the necessary resources for care continuity.

• Psychoeducation: Educate patients and their families about various health conditions and treatment options, aiding in their overall understanding and adaptation.

• Quality Improvement: Contribute to quality assurance initiatives and community improvement projects related to social work services, which influence the clinic's operational protocols.

• Supervision: Provide clinical supervision to Master of Social Work (MSW) students and interns to promote professional development within the field.

In this role, you will engage with a diverse patient population, including adults coping with acute and chronic mental health conditions, geriatrics facing life transitions, and adolescents managing developmental challenges. Qualifications\n

• A master’s degree in behavioral sciences, social work, counseling, or a related field from an accredited institution.

• Valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in California.

• Proven ability to document clinical assessments and compose case narratives clearly and accurately.

• Strong proficiency in applying counseling techniques and social work theories to effectively support patients.

Compensation and Benefits\n
