Key Responsibilities\n

• Conduct thorough assessments and ongoing evaluations to identify the individual needs of patients, tailoring interventions accordingly.

• Facilitate clear and ongoing communication between supervisors, care staff, family members, and other stakeholders regarding patient progress, challenges, and treatment plans.
\n
• Provide structured schedules for appointments, home visits, and observations, ensuring consistency and accountability in therapeutic practices.
\n
• Coordinate necessary in-home visits that meet observational and evaluative conditions, engaging in comprehensive chart reviews and data analysis.
\n
• Monitor the implementation of therapeutic plans to assure effectiveness and fidelity to the diagnosed needs of clients.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing to a multidisciplinary approach to client care.
\n
• Complete detailed monthly and quarterly progress notes to maintain high standards of documentation and accountability.
\n
• Organize and lead training initiatives to ensure compliance with best practices among staff and team members regarding program implementation and documentation.
\n
• Conduct follow-up interviews and home visits to provide continuous support, addressing any emerging needs of clients.
\n
• Maintain thorough and organized records to ensure availability of treatment information for clients, families, and other mental health professionals.
\n
• Act as a community liaison to foster relationships with various organizations and stakeholders for improved patient support and resource accessibility.

• Contribute creative input into developing training materials that enhance program effectiveness for diverse client groups.

In this role, you will work with a diverse client base, including adults and youth facing a range of mental health challenges. Your expertise in family therapy, couples therapy, trauma, or substance misuse will be invaluable in understanding and addressing the multifaceted needs of those you serve. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• A valid and unrestricted Professional Counselor (LPC) license from the state of Colorado.
