Key Responsibilities\n

As a Licensed Clinical Social Worker, your role will encompass a variety of responsibilities. You will:

• Conduct thorough evaluations of patients’ needs, allowing for a tailored and dynamic approach to care that aligns with each patient’s unique circumstances.

• Collaborate closely with physicians and caregivers to develop comprehensive care plans aimed at achieving optimal mental health and wellness outcomes.

• Implement and adapt care plans, ensuring they meet the evolving needs of patients and their families.

• Provide therapeutic interventions, including one-on-one counseling and group therapy sessions, as well as crisis intervention when necessary.

• Facilitate individual psychotherapy sessions and provide psychoeducation to families, enhancing their understanding of mental health issues and treatment options.

• Regularly assess and evaluate the progress of clients, establishing specific goals and adjusting strategies as needed to support their journey toward recovery.

• Demonstrate sensitivity to the diverse needs of patients and families, identifying additional resources and support systems as required.

• Document patient care activities accurately, while ensuring compliance with established policies and mental health standards.

Patient Population / Client Focus\n

Your primary focus will be on patients receiving hospice care, many of whom may be experiencing emotional distress due to life-limiting illnesses. You will support individuals grappling with acute mental health diagnoses related to their medical conditions and help them navigate the complexities of end-of-life care. By providing tailored support, you will enable patients and families to cope with grief, loss, and the psychological aspects of terminal illness.

Qualifications\n

The qualifications required for this position include:

• A Master’s Degree in Social Work from an accredited institution recognized by the Council on Social Work Education.

• A valid social work license as per state regulations, with designations such as LMHC, LMFT, or LICSW.

Compensation and Benefits\n
