Work Environment\n

This role is entirely remote, allowing you to work from the comfort of your own home while maintaining a flexible schedule. Our team operates primarily Monday to Friday from 9:00 AM to 5:00 PM. You'll utilize modern telehealth technologies to connect with patients and collaborate with fellow healthcare professionals, ensuring seamless care delivery.

Key Responsibilities\n

As a Licensed Mental Health Counselor, you will utilize your expertise to:

• Counsel patients in crisis situations, providing immediate support and intervention to assess and treat emotional and behavioral challenges.

• Maintain open lines of communication with physicians to update them on patient progress, address concerns, and collaboratively resolve any issues.

• Document patient interactions accurately and in a timely manner, including clinical and progress notes, adhering to our rigorous departmental policies.

• Work closely with our interdisciplinary healthcare team to develop comprehensive treatment plans, including discharge strategies, tailored to the psychosocial assessment of each patient.

• Engage with community resource groups and represent Empower Well Health Care Center at meetings and on various committees to advocate for addressing patients' social needs.

Patient Population / Client Focus\n

This position focuses on providing mental health support to a diverse patient population within a hospice setting. The patients often face terminal illnesses and significant emotional distress. Your role will involve facilitating group and individual therapy sessions that help them process grief, anxiety, and existential concerns while promoting comfort and peace during their final stages of life.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master's degree in a relevant field such as Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling.

• One of the following independent licenses: Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two years of professional experience in a clinical setting, preferably with an emphasis on crisis intervention and end-of-life care.

Preferred Experience\n

While not required, candidates with prior experience working in hospice or palliative care settings are highly desirable. Familiarity with collaborative care models and telehealth practices will also be a significant advantage.

Compensation and Benefits\n
