Key Responsibilities\n

• Conduct thorough evaluations and comprehensive assessments to understand the needs of patients and their families.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring they are aligned with best practices.

• Monitor and evaluate the outcomes of implemented plans, adjusting strategies based on ongoing assessments and feedback.

• Provide emotional and psychological support to families, equipping them with strategies to cope with the challenges they face during hospice care.

• Engage with families in developing joint plans that utilize evidence-based practices designed for the natural learning environment.

• Ensure full compliance with all state and federal regulations related to clinical practices and patient care standards.

• Promote a healthy learning environment through effective rapport building with children and families, ensuring that all interactions prioritize their emotional well-being.

• Utilize a coaching model to empower families, teaching them strategies that enhance their child's development within the home environment.

Patient Population / Client Focus\n

Our LCSW will primarily work with children undergoing hospice care, along with their families. This includes supporting families with children who have severe medical conditions, disabilities, or developmental delays. The emphasis on fostering a natural learning environment means you will be engaging not only with the child but also empowering parents and caregivers with training and resources to facilitate developmental progress.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited program.

• Current licensure as a Licensed Clinical Social Worker (LCSW) granted by the Department of Public Health.
