Key Responsibilities\n

• Assess client needs through thorough interviews to gather comprehensive information on their psychological condition, strengths, and challenges, thereby individualizing care plans.

• Develop and implement tailored intervention strategies designed to help clients overcome personal difficulties, while coordinating necessary support services.

• Monitor and evaluate client progress, adjusting treatment plans based on measurable outcomes and ongoing assessments.

• Provide crisis intervention to clients when necessary, ensuring prompt and effective responses to urgent mental health situations.

• Maintain meticulous records regarding client interactions, progress, and referrals, and communicate findings during case conferences.

• Actively refer clients to community resources, services, and additional supports to bolster their recovery journey.

• Collaborate with multidisciplinary teams and serve as a key worker to enhance the overall support network for each client.

Qualifications\n
