Key Responsibilities\n

• Conduct thorough evaluations and assessments to determine the needs of young children and their families.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs)

• Monitor and evaluate progress of enrolled families and children as part of a multidisciplinary team.
\n
• Provide evidenced-based support strategies and actionable insights to families tailored to each child's unique situation.
\n
• Create a strong rapport with children and their families, fostering trust and promoting a healthy, engaging environment for learning.
\n
• Implement natural environment practices and coaching models to enhance the learning experiences of children.
\n
• Adhere strictly to all applicable federal and state regulations, ensuring compliance and safeguarding all stakeholders involved in the process.

• Utilize digital tools and resources to maintain accurate records, document client progress and communicate effectively with team members.

Patient Population / Client Focus\n

This role is primarily focused on the youngest members of our community, particularly children from birth to age three who may face developmental challenges. Our approach ensures that these children receive personalized attention and guidance that is critical during their formative years. You will work closely with families, educators, and various healthcare professionals, all of whom play a role in the child's developmental journey.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work is required.

• Must hold a valid license issued by the Department of Public Health as an LCSW.

• A valid driver’s license is required for potential in-person sessions.

• Exceptional organizational and time management abilities to effectively manage caseloads.

• Strong interpersonal and communication skills necessary for collaboration with diverse individuals.

Preferred Experience\n

Compensation and Benefits\n
