Key Responsibilities

• Manage the entire process of therapeutic care for an assigned group of patients, coordinating closely with physicians and the patient care team to ensure optimal outcomes.
• Conduct thorough patient assessments and actively participate in the development and implementation of individualized care plans that focus on patient engagement and goal achievement.
• Deliver or oversee a comprehensive array of physical therapy services, ensuring continuity of care throughout the patient's rehabilitation journey.
• Provide expert evaluations and execute targeted treatment interventions, adhering strictly to established legal and ethical standards of practice.
• Evaluate therapeutic, rehabilitative, and functional status of patients to contribute effectively to the overall plan of care.
• Administer purposeful and skilled therapeutic care within the patient's home, utilizing appropriate procedures and techniques to drive positive changes in condition and function.
• Instruct patients and their caregivers on therapeutic exercises, use of appliances, and prosthetic devices for continued at-home rehabilitation.
• Prioritize your caseload, assess daily staffing needs, and ensure efficient team coordination and hand-offs.

Patient Population / Client Focus

You will primarily work with a diverse patient population, including adults and pediatrics, who may be recovering from orthopedic surgeries, traumatic injuries, or chronic conditions. Qualifications

• A valid license to practice physical therapy.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification or willingness to obtain it within 30 days of employment.
• Passion for patient care; no prior experience is required, making this an excellent opportunity for new graduates as well.

Preferred Experience

• Experience in home health care or community-based rehabilitation is a plus but not mandatory.
• A background in managing diverse patient cases, particularly in orthopedic or post-surgical rehabilitation, will be advantageous.

Compensation and Benefits
