Work Environment

This role operates within a home care setting, where you will perform assessments and administer therapies in the comfort of patients' homes. Key Responsibilities

• Conduct comprehensive evaluations, including detailed histories and assessments, to determine patient needs and establish individualized treatment plans that align with patient goals.
• Communicate evaluation findings clearly to patients and their families, involving them actively in the goal-setting process.
• Implement targeted treatment protocols, adapting approaches based on each patient's condition and response, and remain knowledgeable of evolving healthcare practices.
• Collaborate with fellow healthcare professionals to ensure a multidisciplinary approach to care; promptly document changes in patient conditions that may warrant a revised plan of care.
• Manage discharge planning and communicate follow-up care needs clearly to patients and families.
• Maintain accurate and timely documentation for all patient interactions, including treatment plans, daily notes, interim summaries, and final evaluations.
• Demonstrate exceptional time management skills while meeting productivity expectations and maintaining a reliable schedule.
• Participate in departmental goals by contributing to marketing efforts and community outreach initiatives.
• Engage in lifelong learning by pursuing continuing education opportunities, and mentor students as a clinical instructor.

Patient Population / Client Focus

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred.

Preferred Experience
