Key Responsibilities\n

• Conduct thorough virtual psychosocial assessments to evaluate client's emotional and behavioral health, strengths, needs, and challenges.

• Design evidence-based treatment plans that incorporate client goals and evidenced interventions tailored to their unique situations.

• Lead individual and group therapy sessions utilizing modalities such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Collaborate closely with medical providers, case managers, and other stakeholders to establish integrated care strategies that promote client wellness.

• Serve as an advocate for clients by assisting them in accessing necessary resources, support services, and community programs.

• Maintain meticulous documentation of client interactions, treatment progress, and outcomes in electronic health records, adhering to compliance standards.

• Actively participate in team meetings and case conferences to discuss client progress, share clinical insights, and contribute to quality improvement initiatives.

Patient Population / Client Focus\n

The clientele served at our hospice center primarily consists of individuals facing terminal illnesses, chronic pain, and other mental health difficulties exacerbated by their medical conditions. You will engage with clients from diverse backgrounds, employing cultural competence and sensitivity to better understand their unique perspectives and needs, ultimately fostering therapeutic relationships that facilitate healing.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid and current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• At least two years of post-licensure experience in providing clinical social work services.
